POA Requirements for Individuals
Your document must:
- Show your current residential address
- Be addressed to you and issued within the 36 Months
- Display a physical address — PO boxes and virtual addresses are not accepted
Accepted documents include:
- Bank statement
- Utility bill
- Government-issued letter
- Government-issued ID (issued within the last 36 months)
- Residential lease agreement (must be current, even if older than 36 months)
POA Requirements for Businesses
Your document must:
- Show the business’s current operating address
- Be addressed to the applying entity and issued within the last 36 months
- Display a physical address — PO boxes and virtual addresses are not accepted
Accepted documents include:
- Bank statement
- Utility bill
- Government-issued letter
- Office lease agreement (must be current, even if older than 36 months)
Special note for DAOs:
If a DAO does not have a physical operating address, they may provide the verified control person’s residential POA instead.

- Make sure the text is sharp, readable, and not covered by glare or shadows
